Field Experience - Field and Clinical Experiences
Apply for Student Teaching/Final Internship, Practicum, Observation
You must complete the entire application package in one sitting. You cannot save your work.
Before you begin, ALL students MUST have:
- Course Prefix and Course Number (if your field experience is part of a class)
- Number of contact hours required. Enter “Full-Time” when requesting student teaching or final internship.
- A completed Professional (Tort) Liability Insurance form*
- A completed *Authorization for Release of Records & Information form*
*Click on the links, print the forms, complete, and sign. No electronic or typed signatures will be accepted. Submit the forms by uploading them into the application in Part 1. Keep the original forms for your records.
- The application number of your PreCheck criminal background report.
If you have misplaced your email from PreCheck with your background check application number, please email StudentCheck@precheck.com with your details and they will provide you with another copy.

Note: Reports are considered valid when purchased within one year of the month in which your field or student teaching/internship experience starts.
Students applying for student teaching or a final internship MUST ALSO have:
- The course titles of education courses and major courses that you have completed.
Include current courses by noting In Progress by the course title.
- A list of all observations and practicum experiences that you have completed. Include:
Course Title
Course Prefix and Number
School
Grade Level(s)
Subject(s) you observed and/or taught
You may list current field experiences by noting “In Progress” by the course title
- A very brief narrative essay (thirty typed lines or less) composed by you. Describe your prior teaching, extracurricular, and service experiences that are relevant to your program and to your goal of becoming a teacher; describe your personal qualities or skills that are relevant to your future role as an educator. Type the essay in the space provided on the application.
