PRISM Learning Community Forms
The following forms must be filled out and submitted by
each learning community. Submit the annual learning community description and meeting report forms
to PRISM Evaluatior, Linda Gilbert and NE GA PRISM Director, Dava Coleman. Note that the forms have been shortened for
2007-2008.
- Meeting Report - this form should be filled out at each learning community meeting.
- Annual Learning Community Description - This form should be filled out for each learning community at the beginning of the year. This plan can be updated or changed, depending on the dynamics of the learning community.
- Learning Community Reporting Guidelines - If you are not sure whether or not your meeting should be counted
as a Learning Community, please consult the Learning Community Reporting
Guidelines.
