Forms and Materials


The best way to use any of the forms below is to

  • choose the file format you wish to use,
  • save a copy to your computer, and then
  • open the newly saved file to complete your form.


PC instructions to save your file:

  • Move your mouse over the file name you wish to use.
  • Use the right mouse button to open a dialogue box.
  • Choose Save Link as with your left mouse button.
  • Choose where you wish to save the file and change the file name if you wish.
  • Minimize or close your web browser window.
  • Open the application you want to use.
  • Open the file you saved by going to where you saved your file.
  • Choose the file to open it.
  • Use the file as you wish.


Mac instructions to save your file:

  • Move your mouse over the file name you wish to use.
  • Left mouse click on the file name/link.
  • You will be asked to Open or Save your file.
  • Chose Save File to send to your download location (this may be your desktop or your download folder depending on your browser settings).
  • Minimize or close your web browser window.
  • Go to where you saved your file.
  • Click and drag the file onto the application you are using (for example, Word)
  • Your file will then open so you can use the file as you wish.
