Program Application1. Read all application pages carefully. Be sure that you meet the eligibility requirements before you apply. 2. Please use the Checklist of Application Materials. Include all items on that list when submitting the application. The Cover Page, Project Summary Form, Signature Page of Applicant, Signature Page of Employer, and Budget Format pages should be printed from the pages available on this web site. Following are the application materials in PDF format. *You must have Adobe Acrobat Reader to view the PDF version. If you do not have this FREE product, you may download it here. Cover Page 3. The original application must contain the actual signatures of the designated individuals. Signature pages cannot be mailed separately. Faxed documents will not be accepted. 4. Answer all questions fully, accurately, and succinctly and please type all responses. The application must be signed and dated in the appropriated places. Applications may submitted in English or Spanish. The application may also be duplicated on a word processor, using the same formatting. Limit your replies to the length specified. Submit the original plus five copies of the application. 5. Express mail (for example DHL, Fed Express or UPS) the original application plus five copies to the Cyril O. Houle Scholars in Adult and Continuing Education Program, Department of Adult Education, The University of Georgia, 423 River's Crossing, 850 College Station Road, Athens, GA 30602-4811, U.S.A. A cover letter need not accompany your application. 6. Information contained in this application form may be used in a news release should you be selected. 7. As a result of the Tax Reform Act of 1986, all payments to U.S. citizens are subject to income taxes. Each participant will receive tax information for his/her own records and will be responsible for reporting all scholarship activity to the Internal Revenue Service (IRS). |
