Registration Information

Registration accompanied by full tuition, including agency/vendor purchase order, will be accepted in order of receipt. You may enroll by letter, fax (706/542-4130), or by filling out the on-line registration form. Registrations may be made by phone (706/542-1360), but we request that written confirmation follow. Class size is limited; therefore, it is important to register as far in advance as possible. If you would like to recieve Continuing Education Units (CEUs) or Continuing Maintenance (CM-CRC) for participation in the program, please check the appropriate box on the registration form.

Payment of Fees: Checks should be made payable to the Human Services Management Institute. Full tuition is due at the time of registration. Tuition amount is subject to change.

Tuition: $1000.00 per student (includes technical assistance, course materials and instruction)

Tuition Assistance: The Human Services Management Institute will assist prospective students who do not have the requisite funds to take advantage of our program. One scholarship is set aside for each enrollment period allowing a student to attend at 50 percent off the regular tuition fee. For more information call the Institute at 706/542-1360.

Cancellation Policy: Before a program begins, the Institute incurs substantial costs related to individual registrations. Therefore, registrations cancelled less than four weeks prior to the starting date of the program will incur a charge of 25 percent of the full program fee; within five days of the program, 75 percent will be forfeited. For the convenience of an agency, substitutions may be made at no cost. Participants who substitute must have an approved application.

Technological Assistance: A limited number of state rehabilitation agencies and/or their vendors may require technological assistance in setting- up on-line distance education opportunities. In most instances, the Institute or the UGA Office of Information Technology will be able to assist at no charge. If an agency requires on-site assistance, a reasonable fee for service can be negotiated with the Institute.
