Student Teaching: Student Teacher Evaluation Form
The Student Teacher Evaluation Form is used by field instructors and cooperating teachers to provide mid- and end-of-term evaluations. The evaluations are completed on-line via a url provided by the program. Click on the "PDF Version" link below to download a copy of the Student Teacher Evaluation Form for reference purposes. We encourage you to share your evaluation as a basis for discussions with your student teacher and his/her field instructor. The completed mid- and end-of-term evaluations also provide useful comparisons of both the field instructor’s and cooperating teacher’s perspectives on the progress of student teachers.
