Complete Guide to Graduate School Forms for EPIT Students
Graduate students are individually responsible for completing the following forms, including obtaining all required signatures, and submitting the document(s) to the Graduate School, unless otherwise indicated. For deadlines regarding when forms are due each semester, please refer to the Graduate School website. Additional forms for special circumstances are available through Graduate School Forms & Publications.

Before the end of the first semester (MS & MA students) or year (EdD & PhD students) of residence, the Advisory Committee Form must be submitted to the Graduate School. The committee must consist of a minimum of three graduate faculty members. Consult your Advisor or Program Coordinator for questions regarding any other requirements or restrictions on committee membership. | |

Depending on the degree program, the Program of Study Form may be required to be submitted early, near the middle, or near the end of the program. It should be filed by the second semester of residence (MS & MA students), by the second week of the graduating semester (EdS & MEd students), or prior to scheduling Comprehensive Exams (EdD & PhD students). If the Program of Study needs to be changed for any reason after it has been filed, submit the Recommended Change in Program of Study Form. | |

Doctoral students are responsible for initiating an application for admission to candidacy with the Graduate School at least one full semester before the date of graduation. This application certifies that all requirements for candidacy have been fulfilled. | |

When the major professor is satisfied with the completed thesis or dissertation, he or she will distribute copies of the file to the remaining members of the advisory committee and schedule a final oral defense. Students must notify the graduate coordinator, Dr. Bonnie Cramond, at least two weeks prior to this defense. EdS & MEd students submit the Report of Final Examination to the department, not the Graduate School. | |

The Electronic Thesis and Dissertation (ETD) Submission Approval Form should be submitted with the Approval Form for Thesis/Dissertation and Final Oral Examination. Additionally, students must complete a format check of their thesis or dissertation before submitting the final ETD. | |

The application for graduation must be submitted no later than the end of the second week of the graduating semester (first week for summer graduates). | |

Students who miss filing for graduation by the deadline or have failed to submit any previously required forms have the option of paying a single fee of $50 (check or money order in U.S. dollars) for late processing of all required forms. A completed Late Filing for Graduation Form, all required graduation forms, and the late fee payment must be submitted to the Graduate School Office of Enrolled Student Services within 45 calendar days of the original deadline. | |

Students who are unable to complete degree requirements must notify Enrolled Student Services to change their graduation date to a subsequent term. Failure to submit this form may result in having to reapply for graduation. | |

Doctoral students must submit their dissertation title for printing in the long version of the commencement program. This version contains the names of all candidates, dissertation title, and major professor for doctoral students and names and majors for education specialists and master’s students. | |

There is currently no limit to the number of guests you can invite to the graduate commencement ceremony, and tickets are not required. However, academic attire is required to participate in the ceremony. Caps, gowns, and hoods may be purchased at the UGA Bookstore. You may order graduation announcements online at Jostens. |
