Documentation

If you know what your major will be, check with the program or department of your choice for specific documentation requirements.

However, if you do not have a major in mind, there are two (2) documents you may complete that will comply with departmental requirements.

  1. Interview your supervisor following the instructions described on the Early Community Experiences Supervisor Interview Form (download PDF file) and have your supervisor initial the form for submission.
  2. Complete the Early Community Experience Reflection Form (download PDF file).

Complete the supervisor interview and the self-reflection forms as you perform your Early Community Experiences.

If required by the department or program:

Submit both the initialed Supervisor Form and completed Reflection Form as part of your application for admission into your desired department or program. Keep a copy for your files.

If not required by the department or program:

Keep the completed forms with your files or portfolio. You may choose to review them later to see the growth and progress you have made in your understanding of what it means to be a teacher.
