Application Checklist for CSD Graduate Programs
A complete application to any graduate degree program in the Program of Communication Sciences and Disorders consists of the following. For master’s programs, these materials must be received by January 3 (or the first business day thereafter) for first consideration for the following Fall semester. Ed.S. and Ph.D. applicants, discuss due dates with your faculty sponsor.
- Required, to be sent to the Department (address is on the application):
- Application to the department (which includes a personal statement, described within the application).
- Resume.
- Required, to be sent to the Graduate School or complete on-line (address is on the application):
- Application to the University of Georgia’s Graduate School, including an application fee (online).
- Two official transcripts from each institution of higher education attended except the University of Georgia, must be submitted by the application deadline or sent by you in the format described in the UGA Transcript Request Form.
- Official GRE scores, sent directly to the University of Georgia’s Graduate School.
- Three letters of recommendation, written using the online form available from the University of Georgia’s Graduate School website.
- Optional, to be sent to the Department if desired:
- Additional letters of recommendation
- Application for Financial Assistance/Graduate Assistantship (pdf file – requires Adobe Acrobat Reader)
