Complete the online Graduate School Application form and required documents/fees at: https://www.applyweb.com/apply/ugagrad/checklist.html
Please be aware that the Online Recommendation Option is not available for our department. See departmental instructions below:

- Completed Departmental Application Form
- One official transcript from each institution of higher education attended
- Statement of Purpose
- Vita/Resume
- 3 Letters of Recommendation
- Submit all the departmental materials in one packet to the following address:
