This instrument was developed as part of an ongoing line of research on the effects of the working alliance on psychotherapy outcome. This clinical intake instrument provides information about the client’s expectation of therapy, relationship, and self. Dagley, J. C., & Campbell, L. F. (2000). Treatment Evaluation Questionnaire-Client Form . This instrument was developed as part of an ongoing line of research on client satisfaction with therapy and the therapist in relationship to expectations for therapy. The instrument has a past, present, and future format for multiple data point collection. Dagley, J. C., & Campbell, L. F. (2000). Treatment Evaluation Questionnaire-Therapist Form. This instrument was developed in order to compare therapists’ evaluation of therapeutic outcome with clients’ evaluation of outcome. This instrument is the correlate to the Client Form of the same instrument. The appropriate form of the instrument is given to both client and therapist. Campbell, L. F., & Dagley, J. C. (1999). Common Factors Identification and Evaluation in Case Conceptualization. A series of six stimulus instruments were developed for a longitudinal study of critical thinking in case conceptualization. These instruments are used as training mechanisms for developing higher level cognitive functioning in identifying and analyzing therapeutic cases for treatment planning. Campbell, L. This intake medication form was developed in order to identify prescription and over the counter medications that could affect mental status for counseling and psychotherapy clients. This form is used in the clinic for client intake and has been reprinted in the text entitled, Innovations in Clinical Practice: A Source Book (2002) by Leon VandeCreek.
